Recognition for Commitment to Quality Management

Lyceum of the Philippines University, Manila

Lyceum of the Philippines University (LPU) was founded on July 7, 1952 within the Walled City of Intramuros, where Spanish adventures founded the city of Manila. The school was founded by Dr. Jose P. Laurel, former President of the Philippines. With the philosophy of Veritas et Fortitudo, Pro Deo et Patria (Truth and Courage for God and Country.

Today, the esteemed academic institution prides itself with 28 program offerings, most notable of which are the Tourism and Hospitality Management Programs, being the first university in the Philippines to be granted Full International Accreditation for Hotel and Restaurant Management (HRM) and Tourism (Undergraduate and Graduate Programs) by the International Centre of Excellence for Tourism and Hospitality Education (THE-ICE) and the first university to be granted Level 3 Reaccreditation Status by PACUCOA and certified by the Federation of Accrediting Agencies of the Philippines (FAAP) for HRM Program in the Philippines.

Over the course of the past six decades, LPU has built on the legacy of Academic Excellence and Quality Education as displayed by its founder, Dr. Jose P Laurel. LPU has recently received the Philippine Quality Award for commitment to Quality Management. It has also been granted Autonomous Status, recommended Center of Excellence for Hotel and Restaurant Management, Center of Development for Business Administration, IQuAME Category A Teaching University, and deputized Expanded Tertiary Education Equivalency and Accreditation Program (ETEEAP) by the Commission on Higher Education (CHED), Granted ISO 9001:2008 Certification by Societe General de Surveillance, Granted Level 3 Reaccredited Status for Hotel and Restaurant Management, Business Administration and Liberal Arts Programs by the Philippine Association of Colleges and Universities Commission on Accreditation (PACUCOA) and certified by Federation of Accrediting Agencies in the Philippines (FAAP), First Educational Institution Certified by Investors in People (Bronze Award), Ranked No. 1 Nationwide in Customs Broker Licensure Examinations for 4 consecutive years (2009-2012) and 6 out of 10 topnotchers in 2013, Ranked No. 3 Nationwide in the July 2010 Nurse Licensure Examinations and is now a Probationary Member for the National Collegiate Athletic Association (NCAA) 89th Season

Zamboanga Polymedic Hospital, Inc.

Ciudad Medical Zamboanga (CMZ) ) is a private tertiary, level 2 general hospital located in Zamboanga City. It is owned by the Zamboanga Polymedic Hospital, Inc. (ZPHI), a SEC. registered stock corporation and was opened to the public in October 2003. CMZ’s main product offering is healthcare services in a hospital setting. Starting as a 30-bed hospital in 2003, it is now the biggest private hospital in the region in terms of authorized bed capacity which is 160. While the primary catchment area of CMZ is Zamboanga City, a substantial number of its patients hail from the Zamboanga Peninsula region and the neighboring islands of Basilan, Sulu and Tawi-tawi

The vision of CMZ is a hospital with performance excellence that is nationally recognized. Its missions are: 1) To provide integrated value-based healthcare services; 2) To provide highly competent staff; and 3) To pursue a corporate social responsibility program. Since 2009, CMZ has been using the standards of the Baldrige Health Care Criteria for Performance Excellence with the objective of continually improving its quality management system. CMZ takes pride as the first and only hospital that received the Center of Quality hospital accreditation in Region IX from PhilHealth last 2011; and this 2014, as a recipient of the Philippine Quality Award for Commitment to Quality Management conferred by no less than the President of the Republic of the Philippines, His Excellency Benigno S. Aquino.

Staying true to its mission to provide integrated value-based healthcare services, CMZ has been pro-active and has taken the lead in the acquisition of state-of-the-art technology to complement healthcare services. In 2005, CMZ was the first to acquire the dual-slice CT Scan in the Zamboanga Peninsula and in July 2013, it upgraded its CT Scan to the 64-slice Siemens Somatom Perspective, which incidentally, is the first of the Perspective series in Southeast Asia. CMZ is also the first to acquire a 1.5 Tesla Magnetic Resonance Imaging (MRI) in Mindanao, as well as the first to have the Digital Mammography system in Western Mindanao last October 2012. For its IT-enabled services, CMZ is the first private hospital to acquire Computed Radiography and the PACS system in Western Mindanao, as well as the first hospital to install the hospital information system Bizbox Version 8 in the region. This advocacy of CMZ is relied upon by its many partners, including the Joint Special Operations Task Force-Philippines (JSOTF-P) which has since made CMZ as its healthcare provider. CMZ attributes its dynamism to its more than 500 strong workforce and physicians. As of December 2012, the physician group is composed of 53 active consultants and 162 visiting consultants distributed among the different medical specialities.

During the Zamboanga Siege in September 2013, CMZ manifested the true spirit of volunteerism and passion in supporting its workforce and community as well as its partners from the Armed Forces of the Philippines for their patients with severe injuries. CMZ remained steadfast in this adversity mostly because of the systems and preparedness measures based on the PQA and Balrige Criteria. Corporate Social Responsibility is one of CMZ’s core competencies with Breast Wellness Center as its flagship program. It conducts lectures even outside CMZ and spearheads advocacy campaign in an effort to share to the community the importance of early detection of breast cancer followed by prompt treatment to save more lives. As part of its societal contribution, CMZ (ZPHI) is the second highest corporate taxpayer in Zamboanga City in 2011, and is the only hospital that has affiliated with the local chamber of commerce to help in the setting of economic agenda for the city.

At its tenth year and onwards, CMZ continues its journey towards Performance Excellence under an inspired leadership, guided by strategic planning for a dynamic workforce providing integrated value-based healthcare services, for the delight of the customers within a learning organization. Philippine Association of Colleges & Universities Commission On Accreditation (PACUCOA)

Philippine Association of Colleges & Universities Commission On Accreditation (PACUCOA) is a private, voluntary non-stock, non-profit organization with official address at Suite 7, Mezzanine Floor, Eagle Star Condominium, 25 F. Dela Rosa St., Loyola Heights, Quezon City. The organization started as a committee of the PACU but was established as an independent corporate entity in 1973 and was officially registered with SEC. in 1988. The assets of PACUCOA amount to Php 11, 856, 014.19. The Major asset of PACUCOA are the following: five(5) condominium units - fully furnished with furniture and fixtures, equipment and cash in hand and in bank of P3, 835, 693.94.

PACUCOA is a corporate body, thus it complies with government regulations such as those of SEC, DOLE, SSS, Pag-ibig, Quezon City Government and Barangay Loyola Heights (LGU). Moreover, it does not only comply with the requirements of CHED and DepEd, PACUCOA as an accrediting body adopted standards which exceeded the requirements of both agencies -- the essence of accreditation in the Philippine setting. PACUCOA is recognized as full member of an International QA network, the International network of Quality Assurance Agencies in Higher Education (INQAAHE), a worldwide network of QA bodies compromising of almost 100 countries around the world. The organization has fully complied with the standard of ISO 9001:2008 and was certified under its terms by the Societe General de Surveillance (SGS).

PACUCOA was originally organized to accredit non-sectarian schools, although by virtue of the voluntary nature of accreditation, Catholic, Christian and state colleges and universities have engaged the accreditation services of the organization. The key elements of the performance improvement system are the ISO standards; PQA criteria/standards, staff development; benchmarking; various surveys and evaluation systems, the strategic and operations planning mechanisms, strategic objectives, KRA's, KPI's, awards and recognitions, merit increase, calibration training for accreditors and balanced scorecards. At present PACUCOA has accredited 1089 circular programs based on 150 institutions as of October 2013.

Department of Science and Technology - Region XI

The Department of Science and Technology Regional Office No. XI (DOST XI) is one of the 16 regional offices of the Department of Science and Technology (DOST), the government agency mandated to provide central direction, leadership and coordination of scientific and technological efforts and ensure that the results therefrom are geared and utilized in areas of maximum economic and social benefits for the people. As the focal point for the planning and implementation of science and technology (S&T) programs, projects and services in Davao Region, the DOST XI expands its reach to cover 1 chartered city and 4 provinces namely, Davao Oriental, Compostela Valley, Davao del Norte, Davao del Sur and Davao City.

DOST XI envisions a dynamic and proactive organization committed to excellence in public service through S&T. Guided with its mission to build knowledge and partnership in S&T in support to sustainable socio-economic development in Mindanao, it performs the following major services and products:

 

1. Technology Transfer through management of flagship S&T programs, technology trainings, and consultancy services; and

2. Science and Technology services through the testing (microbiology and chemistry) and calibration services , S&T scholarship, S&T promotion, and research and development (R&D)management

 

DOST XI’s commitment to quality management system has led to certifications on compliance with international quality management standards such as ISO 9001:2008 and ISO IEC 17025. Philippine Information Agency. Our institution was created Executive Order 100 by President Corazon C. Aquino in 1986 to serve as the government’s information and communication arm in engaging the citizenry in nation-building. At present, we have 16 regional offices (RO) and 71 provincial Information Centers (PIC) nationwide.
